Service Description
Adapt PT & Fitness is hosting an adaptive sports camp! This camp is designed for serving young individuals with physical and/or visual disabilities to explore the world of Paralympic sport, refine foundational sport-specific skills, and discover opportunities to compete at regional and national events this summer! The Adaptive Sports Camp will be led by our very own expert in the para sport community, Dr. Jacob Graboski, with support from our certified inclusive fitness trainers! Athletes doe not have to have prior experience to participate. In fact, the newer the better! Athletes can expect to participate in a jam-packed session including warm-up, foundational strength training, sport-specific stations, and parent/coach education to discuss technical rules of sport as desired. Parents are encouraged to attend to learn about regional opportunities to have their athlete compete, with potential to qualify for Move United Nationals! Sports covered include shot put, discus, track, and para powerlifting. Para sports are open to both standing and seated athletes - so come join in on the fun, regardless of how you move! Join Team Phoenix this year as we look to take over the Dairyland Games competition in May, join a growing team, and discover the joys of sport!

Cancellation Policy
APPOINTMENT CANCELLATION POLICY Client understands and agrees that all booked appointments must be cancelled, via written or verbal communication to the staff of Adapt Physical Therapy & Fitness, within 24-hours of the scheduled appointment time. In the event that written or verbal communication of an appointment cancellation is not provided, or a client "no-shows" the appointment without prior communication, a 50% cancellation fee may be applied to your account at the full discretion of Adapt Physical Therapy & Fitness. We understand that life events and emergencies may arise, so we will do our best to reschedule, if possible, to avoid any cancellation fees as able. NO SHOW POLICY Client understands and agrees that arrival to appointments greater than 10 minutes late without notification to Adapt Physical Therapy & Fitness LLC will NOT guarantee that services can be provided, and the appointment time may need to be re-scheduled with a cancellation fee incurred. Should the Client accrue more than 3 missed appointments without proper notification (“no-show”), Adapt Physical Therapy & Fitness reserves the right to cancel future appointments and/or discontinue all future services for the Client. The Client will be notified in writing if Adapt Physical Therapy & Fitness LLC elects to discontinue services due to frequent no-shows, and the Client will be required to obtain written approval from the Owner of Adapt Physical Therapy & Fitness in order to resume their services. PAYMENT & BILLING AGREEMENT Client understands and agrees that Adapt Physical Therapy & Fitness will be paid for all services rendered to the client according to the current fee schedule as designated by Adapt Physical Therapy & Fitness, which is available to the client upon written request. It is the sole responsibility of the Client, or the approved guardian/guarantor, to pay for services rendered by Adapt Physical Therapy & Fitness. The Client may request copies of receipts of services, documentation, and/or billing codes for end-of-year tax and health insurance-reimbursement purposes (HSA, health insurance reimbursement for “out-of-network” provider, etc.). It is the responsibility of the Client to make full payments within 30 days of receiving a bill or invoice, unless otherwise indicated by Adapt Physical Therapy & Fitness LLC. If you are unable to pay your bill within 30 days or your account becomes overdue, a 2.5% interest will be added monthly to your existing account.
