Dr. Jacob Graboski, PT, DPT

Dr. Jacob Graboski, PT, DPT

Owner & Founder, Doctor of Physical Therapy

Dr. Jacob Graboski grew up in Stevens Point, Wisconsin and attended the University of Wisconsin-Madison where he earned a Bachelor of Science in Kinesiology in 2016, and a Doctorate of Physical Therapy (DPT) in 2020. Jacob is a currently a licensed physical therapist and dry needling specialist with expertise in treating persons of all abilities for sports injuries, neurological conditions (Parkinsons, spinal cord injury, cerebral palsy, multiple sclerosis, etc.), vestibular or dizziness conditions, chronic pain, and more! Jacob places emphasis on individualizing each treatment program to meet the specific needs of each patient, while ensuring each patient gains improved access to local community-based sport, fitness, & recreation programming for long term success. Jacob founded Adapt Physical Therapy & Fitness in 2021 with the aim of improving the health & well-being of his clients through compassionate listening, individualized assessment & plan of care development, and a commitment to improving the healthcare experience in a safe, adaptable, and encouraging environment.

Karen Carney, PT, MSPT

Physical Therapist

Karen Carney, MSPT is a Wisconsin native and has lived in Sparta with her husband David and their four children since 2004. She is a 1998 graduate of UW-La Crosse where she earned her Master of Science of Physical Therapy degree. She has held orthopedic certified specialist credentials from 2004 – 2024 and has advanced training and certification in treatment of dizziness and vestibular disorders since 2004. Karen has  experience in treating patients of all ages with special interest in orthopedic conditions, geriatrics, balance disorders, and vestibular dysfunction. Karen values comprehensive treatment of patients aiming at improving function, mobility and pain relief. Patients can expect individualized evidence-based care and management of their diagnosis.

Alexa Lang, CPT, PN Nutrition Coach

Fitness Instructor, Nutrition Coach

Alexa is currently pursuing a degree in Food and Nutrition Science with a minor in Coaching at the University of Wisconsin–La Crosse, with plans to graduate in December 2025. She’s passionate about continuing her education by earning a master’s degree in Dietetics to further support others on their health journeys, which she will be starting at Viterbo in May of 2026. With several years of fitness training experience, Alexa has developed a deep love for helping people not only improve their physical health but also their overall well-being. Recently, she’s found a special passion for working with individuals with disabilities, focusing on creating inclusive and supportive wellness opportunities. Outside of the gym and classroom, Alexa enjoys traveling, going on walks, working out, spending time outdoors, hiking, and making memories with her family and friends.

Addie Pauling, CTRS

Fitness Instructor, Therapeutic Recreation Specialist

Addie is currently studying her Master of Science in Occupational Therapy at the University of Wisconsin-La Crosse, graduating in spring 2027. She is a certified recreational therapist through the National Council for Therapeutic Recreation Certification. She has years of experience working with all different populations, and loves nothing more than helping individuals of all abilities and ages reach their goals. Her appreciation for fitness and wellness stems from her previous years playing competitive soccer and assisting with adaptive sports. This motivates her to continue finding ways to make workouts fun! When Addie’s not in the gym, she enjoys running, reading, and hiking.

Jonah Zoschke

Fitness Instructor

Jonah grew up in Neillsville, Wisconsin, and is currently an undergraduate student at UW-La Crosse majoring in Exercise and Sport Science on the pre-professional track. He will graduate in May 2025 and continue his education in UWL’s Doctor of Physical Therapy Program. Jonah is passionate about creating inclusive sport and fitness opportunities for individuals of all abilities and backgrounds by adapting exercises, providing individualized support, and fostering a welcoming environment where everyone can thrive. In his free time, he enjoys spending time outdoors, attending concerts, and reading.

Addy Walton

Fitness Instructor

Addy grew up in Cottage Grove, MN, and recently graduated from UW-La Crosse with a Bachelor's degree in Exercise and Sport Science. She is currently pursuing a Doctorate of Physical Therapy at UWL where she hopes to continue to help others optimize movement and achieve their health goals. Addy is passionate about promoting overall health and wellness by developing personalized workouts that are tailored to the unique needs and interests of the individual. In her free time, she enjoys spending time with her family and friends, hammocking in the bluffs, and trying new restaurants.

Lauren Franek

Fitness Instructor

Lauren grew up in Mauston, WI and is currently pursuing a Doctorate of Physical Therapy degree at UW-La Crosse. Lauren has a deep passion for creating workouts based around every individual’s needs to promote long term wellness and empower individuals to build healthy habits. She believes individual’s personal growth is the most rewarding outcome in this field. In her free time, she enjoys camping, baking, and spending time with friends and family.

Allie Filzen

Fitness Instructor

Allie grew up in Winona, Minnesota, and recently graduated from Minnesota State University, Mankato in July 2025 with a Bachelor’s degree in Therapeutic Recreation. Allie has been active in adaptive sports for over 11 years, and has been influential in the recreation of her 18 year old brother, who was born with Spina Bifida. Allie has a deep passion for sharing the power of recreation and making it accessible to everyone. Her goal is to help everyone discover their full potential and improve quality of life through movement and recreation. In her free time, Allie enjoys being outside, exploring new places, lifting weights with her brother, and hunting for unique thrift finds.

Gretta Cieslak

Administrative Assistant

Gretta is a Doctor of Physical Therapy student at Rosalind Franklin University, graduating in winter 2026. At Adapt, she supports the team by creating resources, managing social media content, and assisting our expert trainers so they can focus all of their attention on the clients! With years of experience working with individuals with disabilities, Gretta is passionate about helping Adapt athletes succeed. Outside of the gym, she enjoys hiking, baking, and crafting.
